General
Eligibility Requirements for All Funds
- Applicant must have
income at or below 150% of the federal government's poverty level,
except for Warmth and Light Fund.

Applicants's name
and address on the MAAC Standard Intake Form (SIF) must match those on the utility bill.
- Applicant and
agency intake staff member must sign confidentiality agreement on
the SIF.
- All utility
assistance grants should be entered into MAACLink as soon as
possible, but no later than 10 days from the intake date on the SIF.
- The date on the utility
bill must be within 30 days of the date on the SIF.
(If older, the utility company can be contacted for a revised copy
of the bill with a more current date.)
- Client cannot receive
assistance from the fund from two different agencies in the same
year.
- Client
can receive assistance twice in one year from each fund.
- The amount of
the assistance is equal to or less than the amount of the bill.
- No agency
personnel may receive MAAC-managed funds from the agency by which
they are employed.
- The
completed SIF (preferably the printer version from MAACLink), a copy
of the utility bill, and income documentation must be kept on file
at the agency for a minimum of 3 years after the date of service.
Agencies may, at their discretion, keep additional documentation
relative to agency requirements on file. At any time, MAAC may
request to audit agency documentation to ensure compliance with the
program requirements.
